Recalled equipment: what West Virginia requires
The requirement
Only use furnishings, equipment and supplies that meet the standards of the Consumer Product Safety Commission (CPSC) and shall not use any product recalled by the CPSC;
What kind of duty this is
A standing prohibition. Your state says you may not have recalled items on the premises. It does not require you to run a check on a schedule, and there is no state form.
Your state prescribes no recall form. Anything we generate for you is a convenience record for your own files — it is not a state form, and we will not pretend otherwise.

Worth knowing
CENTRES ONLY, and it is a USE prohibition rather than a check mandate — the rule specifies no interval, so do not claim West Virginia requires a periodic recall review. Family child care (78 CSR 18, 19, 20) is a VERIFIED NEGATIVE: those rules carry no general recall duty at all. The state’s filed PDFs are image-only scans with no text layer.
